The Foundation only accepts requests for funding through our on-line application process. Please read below for information on how to apply.

Toolkit

When submitting an application, you will be required to attach additional forms.  Below are links to examples of these required documents; they are intended to be used as guides, but you may use these forms if you prefer.  If you have this information in another format be sure your document contains all of the information requested in our examples.

Required Forms for all Applications:

  • Board of Director’s List - include names, addresses, phone numbers, board position, and occupations.
  • IRS Determination Letter – letter from IRS confirming your organization as a 501(c)(3) Public Charity.  This attachment is not required if your organization is a government entity or tribe.
  • Budget – organization budget and the total project budget, including expenses and anticipated revenue sources in order to fully fund the project.

Additional Form for Technical Assistance Applications:

  • Scope of Work – details the work to be completed with the contracted consultant and anticipated results.

Additional information required for Invited Proposals (Stage 2) only:
If you are invited to complete Stage 2 of the application process, you will be asked to provide the following information at that time:

  • Updated, detailed project & organization budget
  • Statement of Financial Position (Balance Sheet) for the past two years; this outlines your assets and liabilities at a particular moment in time.
  • Statement of Revenue & Expenses (Income Statement) for the past two years; this outlines your finances over a period of time.
  • A copy of your most recent 990 tax return.
  • A copy of your annual financials, audited financials preferred, along with a copy of the management letter from your accountant.
  • Attachment 1 Funding Form - This details all funding sources committed, in-kind, pending and planned.  Please note, you must use this form in our format. 
  • Public Convening Spaces Inventory List - Inventory of existing convening spaces in your community (this is required ONLY on grant applications for public convening spaces).
