When submitting a grant application you will be required to provide additional information about your organization and project through several attachments (see list below). Unless otherwise specified below, you may submit this information in a document that works best for you (as long as it includes the same information as our form). You also should review "Prepare to Complete Your Grant Application."

Required Attachments


Program Area Grants

Board of Directors: Include names, board position, profession, contact information, ethnicity/race and gender. You may use your own form as long as it includes the same information as our form.

Budget Form: Both an organization budget and project budget, including expenses and anticipated revenue sources to fully fund the project/program. You may use your own form; however, it should include comparable revenue and expense categories as our sample form.

Funding Status ReportThis report will provide information on the amount you are requesting per year, your sources  and types of revenue. You must use our form.

Financial Statements: Include a Statement of Financial Position (Balance Sheet) and Statement of Activities (Income Statement) for the past two fiscal years.


Good Neighbor Grants
